Tennessee Rural Partnership has partnered with the Tennessee Hospital Association to sponsor a recorded webinar to provide Tennessee physicians the opportunity to hear details about TennCare’s role in Health Care System and Payment Transformation and how it will impact primary care providers. Dr. Vaughn Frigon, Chief Medical Officer for the Tennessee Health Care Finance and Administration Bureau of TennCare is the presenter.
Primary care transformation focuses on the role of the primary care provider in promoting the delivery of preventive services and managing chronic illnesses over time. The initiative is developing an aligned model for Patient Centered Medical Homes (PCMH), Tennessee Health Link for TennCare members with the highest behavioral health needs, as well as a shared care coordination tool that includes hospital and Emergency Department admission, discharge, and transfer alerts for attributed providers.
The webinar presents a review of TennCare’s role in Health Care System and Payment Transformation and how it will impact primary care providers. This involves a discussion of both Episodes of Care and Patient Centered Medical Homes. The goal is for each participant to understand the direction in which the state’s health care reform is moving and how it will impact their practice.
